About Us

Los Palos Post-Acute Care Center has been serving the community for over 40 years providing compassionate and personal care 24 hours a day. We specialize in providing post-acute rehabilitation services on-site to help our residents regain their independence and return to their homes.

We will provide an interdisciplinary approach to patient care. We put an emphasis on staff training and development so that we employ the most outstanding people to care for your loved ones.

The skilled nursing staff at Los Palos Post-Acute Care Center is available around the clock, every day of the year. Our staff includes Registered nurses, Vocational nurses, Case management, and Certified Nursing Assistants.

Each resident in our facility is seen by members of our nursing team several times a day. Our nursing stations are in close proximity to resident rooms and are staffed at all times.
