The social service team at Los Palos Convalescent Hospital, a Post-Acute Care Center, helps families and residents when they are going through a transitional period.
Our services include:
- Psychosocial and Psychiatric Evaluation
- Individual therapy
- Transportation to appointments
- Accompanying the resident to appointments upon request
- Power of Attorney
